What is MyChart?
MyChart is a platform that enables patients to contact with their UTMB medical team and access personal medical information and clinic services online. This is a free service provided to patients who receive care from UTMB providers by UTMB Health as a convenience and courtesy. Only non-urgent matters should be handled using MyChart; never during an emergency.

What equipment do I need to access MyChart?
A current web browser and internet connection are required.
Once you’ve activated an account, you can use MyChart from your Apple or Android smartphone as well! You can get the MyChart app for free from your app store.
How do I sign up?
Patients at UTMB Health who are 14 years of age or older can open a MyChart account.
Options For Patients Ages 14–17 Years To Register:
Utilization of UTMB Health MyChart must be activated during a clinic visit for our teen patients, ages 14 to 17. We encourage any of our teen patients who are interested in enabling MyChart access to bring up the subject with their physician at their upcoming appointment.
Options For Patients 18 Years And Older To Register:
Ask a nurse or doctor to assist you in activating an account when you sign up at your subsequent clinic visit!
When checking in, ask the front desk Patient Services Specialist for an access code. Alternatively, you can locate your access code after your appointment on your After Visit Summary (AVS). On the MyChart Signup Page, use this code to activate your MyChart account.
An access code and instructions will be emailed to the email address you give once you request an account on the MyChart website.
You’ll need your access code and Medical Record Number to activate your account online (MRN). You must make a MyChart ID, which you must use each time you use MyChart. Make careful to come up with something memorable. Once created, your username cannot be changed.
You will enter a security question and answer and generate a unique password for yourself that you should keep to yourself. These exist to safeguard your private medical information. Finally, you’ll be prompted for your email address.
My access code does not work. What should I do?
The access code you were given has a 90-day expiration date and can only be used once for your security. You must input the access code in capital letters and exactly as it is supplied because it must be. This code becomes invalid after your MyChart account is activated and can be securely thrown away.
